The PICO framework
Four elements that turn a vague clinical concern into a question you can actually search for, and a search strategy that follows automatically.
PICO exists to solve a specific problem: clinical questions arrive vague. "Does exercise help depression?" cannot be searched systematically, because it does not say which patients, which exercise, compared with what, or measured how.
Break it into four elements and both the question and the search strategy appear at the same time.
The four elements

The comparison is optional in practice. Plenty of questions have no meaningful comparator, and leaving C empty is legitimate provided you say so.

Does exercise help depression?
Broken into PICO
P: Adults aged 18 to 65 with a diagnosis of mild to moderate depression
I: Structured aerobic exercise programme, minimum eight weeks
C: Usual care, meaning antidepressant medication alone
O: Change in depression severity, measured by PHQ-9 or BDI-II
The answerable question
In adults aged 18 to 65 with mild to moderate depression, does a structured aerobic exercise programme of at least eight weeks, compared with usual care, reduce depression severity as measured by validated symptom scales?
Longer, and every clause earns its place. That question tells a reader exactly what would count as an answer.
Turning PICO into a search
This is the part that makes the framework worth using. Each element becomes a block of synonyms, combined with OR. The blocks are then combined with AND.
Block 1: Population

Two techniques worth applying throughout. Truncation with an asterisk catches word variants:depress* retrieves depression, depressive and depressed. Quotation marks force an exact phrase, so "physical activity" does not return every paper containing both words separately.
Add database-specific subject headings where they exist. MEDLINE has MeSH terms and CINAHL has its own headings; combining them with free-text terms materially improves recall.

When PICO does not fit
PICO assumes an intervention and a measurable outcome. Qualitative questions have neither, and forcing them into the structure produces something distorted.

Using PICO in your write-up
State the framework explicitly in your methods, present your elements in a table, and show how each became a search block. It takes half a page, demonstrates a systematic approach, and gives your inclusion and exclusion criteria a visible logic rather than an arbitrary one.
